Our home health care support team will develop your Plan of Care jointly with your health care team. We are an approved network provider for most Insurance Carriers and have flexible payment options. We provide our Supplemental Staffing to all home health care facilities.

At Accessible Home Health Care, all of the care we provide is special and important to our patients and us. Certain levels of care required by our patient’s needs require caregivers with special training and certifications. Outlined below are some of these specialty services:

Alzheimer & Dementia Care
Alzheimer's is one of the most increasingly diagnosed diseases. It requires Alzheimer Care Certified caregivers to care for these patients. We assign specially trained care providers to properly care for these patients.

A majority of the five million currently diagnosed patients live at home and a loved one is their primary care provider. As the disease progresses through the 8 stages, it becomes progressively more difficult for the primary caregiver/loved one to continue to care for the patient. The strain on this care provider and patient can be detrimental to the overall well being of both. There should be no guilt for the primary care giver to seek professional assistance from Accessible.

Our caregivers care for the patient and involve the family in this process. We further keep loved ones, not residing with the patient, informed on the patient’s condition either by telephone or email, depending on his / her condition.

Cardiac Care
Each cardiac care patient requires specialty care during the convalescence period and on a continuing basis thereafter. We are in consultation with the physician and therapist to customize a Plan of Care for each stage of recovery. Due to the unique nature of each cardiac care patient’s condition, we customize an individual Plan of Care for each person.

Our Plan of Care is inclusive, and coordinated by a team comprising of a patient’s primary care physician, cardiologist, RN, therapist, and case manager. When continuing care, the caregiver, concentrate on educating the patient on exercising and healthy eating habits.

Post-Op Surgery Care
When you are discharged from a facility after an elective or non-elective surgery, (for joint replacement, etc.) you will require assistance with wound care, therapy, and with daily living and hygiene activities. Our care giving team of RN’s, aides, & therapists will work with your physician and create a Plan of Care to expedite your recovery in the comfort of your home.

Muscular Disability
Muscular disabilities caused due to stroke related paralysis, Parkinson’s, or Multiple Sclerosis restricts ones mobility and requires assistance with, hygiene, travel, cooking, eating, and other daily living activities. Our caregivers are specially trained and experienced in these needs and fully understand the frustration our patients feels due to their condition and experienced in providing care under these conditions while respecting our patients pride and independence.

Wound Care
Caring for a wound in the home is the preferred option by patients. The elimination of having to travel to a hospital or physicians office reduces stress and expedites recovery. It is one of the most utilized home health care skilled services. It is very important to have a fully trained experienced RN to care for the patients. Our wound care specialists have extensive training and experience in this field and are required to stay current on continuing education advancements...

Neonate Home Care
In this age of technology, neonate in home care is a desired alternative. We work with a team of dedicated professional care providers and monitoring equipment to care for neonates and pediatric patients. The caring for babies in the home increases the mother and fathers involvement in care of their baby. This expedites the comfort level and confidence of the parents.

Healthy Baby Care
We understand the responsibility of having a newborn, especially when a mom just needs a well-deserved break or when her new daily routine becomes overwhelming. Our trained caregivers will assist you with caring for your baby while providing mom with the time to do her errands or just take it easy and relax.

Primary Caregiver Respite Care
Being a primary caregiver for a loved one can be stressful and often debilitating. As the person providing the care, this leaves precious little time for you... While our caregiver tends to your loved one, you will have the time to recharge, run errands or just do something for yourself,

Senior Health Monitoring
Rather then be a bother most parents will remain silent rather then complain. Our Senior Monitoring Service allows us to be your eyes and ears with your loved senior family members. We will visit your loved ones in their homes or in a facility and observe their living conditions, hygiene, medicine, food in the refrigerator and their mental and physical condition. We will submit a complete report to you within 24 hours of the visit or sooner if the situation merits immediate action.
